We have some additional information to share with you about your Student Injury & Sickness Plan, as a follow-up to our email on March 26.
Option to Extend Coverage
You have the option to extend coverage of your insurance plan from 10 months (ending June 14, 2020) to 12 months (ending August 14, 2020).
If you are interested, please contact the Health Center to receive a quote for the cost difference of the extended coverage. The cost difference would be billed to your Smart Tuition Account.
How to Manage Coverage and Claim Submissions:
If you have not already, please establish an account with the UnitedHealthcare online system. Establishing an account will enable you to:
- Print, view and request an ID card
- Review claims status and Explanation of Benefits
- Submit claims
- Locate a provider/pharmacy (for students still in the United States)
- Review coverage information and confirm dates of current coverage
- Review/print a copy of your plan summary
How do I establish an account?
This link: https://shipsignup.com/resources/ provides a video on how to set up an account on the UnitedHealthcare website. Once you have watched the video, please email email@example.com or call 1-844-232-7370 (between the hours of 7:30 a.m. EDT and 4:30 p.m. EDT) and a UnitedHealthcare representative will help you complete the process.
How do I submit a claim?
You can submit a claim through your online account, or you can use this claim form.
How do I check the status of a claim?

How will a claim be paid?
While UnitedHealthcare is willing to pay non-U.S. providers, most of those providers will require the student to pay them and then have the student seek reimbursement from UnitedHealthcare. Please note that the check will be made payable to the student in U.S. currency (after currency conversion has taken place). A check will be mailed to the address indicated on the claim form.
