2017-18 New Student Info & Forms

As the spring and summer progress, check this page often for new announcements, forms, and the information you need as you prepare to bring your child to St. Andrew's this fall. (We will also email new students and parents with this information and required forms as they become available.)



You can now find your child's summer reading list and enrichment opportunities on the Library website.

Students are required to read our 2017 all-School read, Colson Whitehead's The Underground Railroad. In addition, students are asked to read three other books from the required reading list (available under the "Required Summer Reading" tab), all of which have been "nominated" by St. Andrew's faculty. There is no summer homework beyond the required reading, but fun enrichment resources can also found at the above link, if you wish to keep in practice in your favorite subjects for the fall.

Our Afternoon Sport & Activity Sign Up Form is now open for incoming students. All students must be enrolled in an afternoon sport or activity for the fall, winter, and spring terms. In addition, all students must meet the following athletic requirements in order to graduate:

  • All Students must participate in a sport of activity each afternoon
  • Freshman and sophomores must participate in at least two sports per year.
  • Juniors and seniors must participate in at least one sport per year.

Please submit this form by June 15.

All incoming St. Andrew's students must submit all completed original hard copies (not photocopies) of their health forms to the Health Center by June 30, 2017. Please contact the Health Center immediately if you think you cannot meet this deadline.

  • Must be printed, filled out by hand, and mailed.
  • Some forms require a physician's signature. All students are also required to receive a physical examination between April 1, 2017 and June 30, 2017.
  • Due back to us by June 30, 2017.

On Opening Day, our health staff will be available in Founders Hall to meet and greet parents and students. During that meet and greet, we require all students, along with their parents or guardians, to check in with the Health Center staff to verify that all health forms are complete and to turn in any prescription medications.

Any pending health forms, insurance cards, or required information that was not previously submitted to the Health Center must be handed in to the nurses at that time.
